At LDDI, we welcome referrals from both family physicians and specialists. Our experienced team of specialists and nurses is dedicated to patient-focused care that promotes a smooth experience, fast recovery, and excellent outcomes.

All of our services are fully covered by OHIP.

All patient referrals must be sent by a primary care provider or specialist and can be faxed to our office at (226) 289-0281.

Once we receive the referral, our team will contact the referring provider's office, and we will contact the patient if an appointment has been scheduled.
