LDDI Anesthesia Questionnaire

If you are scheduled for any endoscopic procedure, you MUST fill out the Patient Anesthesia Questionnaire prior to your procedure. Please fax the completed form to (226) 289-0281 or email a scanned copy to info@lddi.ca. A copy of this form should be brought with you to your appointment unless specified otherwise by our office administrators.
